Integrated care is a term that has no specific legal meaning. The concept of integrated care is applied in a diverse range of settings, ranging from the integration of teams within a single organisation, for example where community services have been transferred into a hospital trust, to the co-location or joint management of health and social service teams, to the design and delivery of new clinical pathways across multiple providers.
