Named clinician for vulnerable – Hunt

Every vulnerable older person will have a named clinician responsible for their care outside of hospital under plans announced by Health Secretary Jeremy Hunt. In a Department of Health (DH) consultation, he states that this is just one way to ensure NHS and social care services work together more effectively for the benefit of patients, both in and out of hospital. The DH argues the move will ensure accountability is clear and clear packages are personalised and tailored around individual needs. Mr Hunt said: We need radical improvements to make sure our most vulnerable and elderly have the support they need to keep them in better health and out of hospital, with one clinician responsible for their care in the community, just as there is a consultant responsible for them in hospital.’
